Very good thing to consider. I think it also depends on how you plan to use the data from the survey. I liked capturing the data in form format because then I could incorporate their responses and deliver relevant content in follow-up communications. But if the objective is to gain an understanding through trends then a survey tool is probably much better at delivering that info.
